Mundtrockenheit
Wer mit Fieber erkältet im Bett liegt oder eine durchzechte Nacht hinter sich hat, kennt Mundtrockenheit. Und weiß, dass sie wieder verschwindet.
Doch jeder vierte Deutsche muss ständig mit diesem Problem leben ? und das kann zur quälenden Dauerbelastung werden. Denn bei Mundtrockenheit (Xerostomie) können die Speicheldrüsen nicht genügend Speichel produzieren, die Schleimhaut im Mund trocknet aus. Die Folgen sind unangenehm und oft schmerzhaft:
- Zungenbrennen
- Entzündung der Mundschleimhaut
- Zahnfleisch- und Zungenbluten
- Mundgeruch
- Durstgefühl
- Trockenheit von Lippen-, Mund- und Rachenschleimhäuten
- Schluck-, Sprech- und Geschmacksstörungen
Das kann dahinter stecken
Die Ursache für Mundtrockenheit herauszufinden, ist oft langwierig und lässt Ärzte und Patienten verzweifeln. Denn sie ist nur ein Symptom, und viele Auslöser kommen dafür in Frage.
Ältere Menschen leiden weit häufiger unter Xerostomie als jüngere, weil die Speichelproduktion im Alter weniger wird. Nehmen sie dann auch noch Medikamente ein, kann sich die Mundtrockenheit doppelt bemerkbar machen. Denn sie ist Nebenwirkung sehr vieler Medikamente. Bei mehr als 400 Präparaten allein auf dem deutschen Markt verweist der Beipackzettel auf Mundtrockenheit als nicht erwünschte Reaktion bei Einnahme der Arznei.
Als Nebenwirkung einer Strahlen- oder Chemotherapie tritt Mundtrockenheit fast immer auf. Dann bleibt sie meistens auch dauerhaft bestehen.
Es kann sich aber auch eine andere bisher noch nicht erkannte Krankheit dahinter verbergen. Denn sie tritt als Symptom einer Vielzahl von Erkrankungen auf, z. B. bei Diabetes mellitus, Schilddrüsenerkrankungen oder Rheuma. Auch diverse Erkrankungen des Zentralen Nervensystems, z.B. Depressionen, werden von Mundtrockenheit begleitet. Emotionale Faktoren (Angst, Stress, Aufregung) können ebenfalls Auslöser sein. Außerdem kann es bei Menschen mit behinderter Nasenatmung, die häufig durch den Mund atmen, zu vermehrtem Speichelverbrauch und infolgedessen zu einer Austrocknung der Mundschleimhaut kommen.
Symptome und Ursachen
Insbesondere als Nebenwirkung von Medikamenten ist Mundtrockenheit häufig. Ein trockener Mund kann aber auch Symptom bei systemischen Erkrankungen wie z.B. dem Sjögren-Syndrom sein. Mundtrockenheit und Zungenbrennen sind darüber hinaus typisch nach ausgedehnter Strahlentherapie im Kopf-Halsbereich z.B. bei Kehlkopfkrebs, aber auch bei Patienten an der Dialyse.
Mundtrockenheit ist eine ernste Störung und für die Betroffenen oft sehr quälend. Beschwerden beim Schlucken, Essen und Sprechen können das tägliche Leben der Betroffenen massiv beeinträchtigen. Deshalb gilt: Rechtzeitig handeln, wenn die Spucke wegbleibt.
Was kann man selbst tun?
Wenn der Arzt aber nichts findet? Dann bleibt dem Patienten nur, sein Leid zu akzeptieren ? und eine symptomatische Behandlung. Das heißt, die Befeuchtung der Mundhöhle wird durch Flüssigkeitszufuhr verbessert. Häufig und viel trinken sowie Mundspülungen mit Wasser, schwarzem Tee oder Olivenöl können kurzfristig helfen, ebenso wie das Lutschen von Eiswürfeln. Oft aber ist Speichelersatzflüssigkeit die einzige Möglichkeit, um die Mundhöhle effektiv feucht zu halten.
